xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles.
These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
Remedy
Toyota
will notify owners. Depending on the vehicle model, dealers will replace the driver or front passenger air bag inflator or the air bag assembly using an alternate inflator , free of charge. The recall is expected to begin November 15, 2019. Owners may contact Toyota

| Lexus is developing an integration within the Service Smarts for Retention (SSR) system to provide unrepaired VINs and customer contact information to dealers.

| A3: | Yes, each time a dealer accesses the data in SSR, they will be required to agree to a series of terms and conditions. We recommend you contact your dealer to schedule an appointment in advance to confirm parts availability and minimize your inconvenience. Why am I receiving another recall notice if I already had the recalled inflator replaced?
Lexus previously recalled these vehicles for this same condition. At that time, the inflator used as a replacement was a new one of the same design (so this was a “like for like” replacement). However, the replacement inflator now in your vehicle can still degrade over time as noted above and must be replaced.
What will Lexus do?
Lexus dealers will replace the front passenger airbag inflator or airbag assembly, depending on the model, with one of improved design FREE OF CHARGE to you.

AMENDED DEFECT INFORMATION REPORT

This supplements Toyota
’s Part 573 Report (19V-741) concerning the front passenger air bag assembly issue on certain Toyota
and Lexus vehicles.
The subject recall affects certain Toyota
and Lexus vehicles equipped with front passenger or driver air bag inflators as a “like-for-like” replacement under a prior recall which contain a non-desiccated, phase stabilized ammonium nitrate propellant. After the amended DIR filing on March 9, 2020, Toyota
has developed effective methods to replace only the affected sub-components of the air bag assembly. Thus, Toyota
is supplementing its Part 573 Report (19V-741) to include the option, where applicable, for replacement of only the front passenger airbag sub-assembly and inflator.
The Description of Corrective Repair Action is revised as noted below:

Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator.

The subject recall affects certain Toyota
and Lexus vehicles equipped with front passenger or driver air bag inflators as a “like-for-like” replacement under a prior recall which contain a non-desiccated, phase stabilized ammonium nitrate propellant. After the amended DIR filing on January 8, 2020, Toyota
identified 17 additional vehicles that may have been equipped with the subject front passenger air bag assemblies. Thus, Toyota
is supplementing its Part 573 Report (19V-741) of January 8, 2020 to add these vehicles to the affected population.
The total number of vehicles potentially involved (increase) are revised as noted below. The production period of each involved vehicle are not affected by this change.

- Percentage of Vehicles Estimated to Actually Contain the Defect:
Toyota
is unable to provide an estimate of the percentage of vehicles to actually contain the defect. As noted in the May 2016 Amendment to the Takata Consent Order, the length of time that a non-desiccated frontal Takata PSAN inflator will have reached the point where they pose an unreasonable risk to safety could be quite long and is dependent on the age of the inflator and environmental exposure, among other factors.
- Description:
The subject vehicles are equipped with front passenger or driver air bag inflators as a “like-forlike” replacement under a prior recall which contain a non-desiccated, phase stabilized ammonium nitrate propellant. According to the Part 573 Reports submitted by Takata on January 2, 2020, a defect related to motor vehicle safety may arise in the inflators due to propellant degradation occurring after prolonged exposure to high absolute humidity, high temperatures, and high temperature cycling. Activation of a non-desiccated ammonium nitrate inflator with degraded propellant may result in an inflator rupture. An inflator rupture may cause metal fragments to pass through the airbag and into the vehicle interior at high speed, which may result in injury or death to vehicle occupants. In consideration of a sufficient supply of final remedy inflators, and upon discussion with the Agency, Toyota
previously accelerated the launch of the remedy for these vehicles in advance of the Takata decisions under this Recall 19V-741. Takata’s January 2, 2020 report prompts this amendment for certain Toyota
and Lexus vehicles.
- Chronology of Principal Events:
October 11, 2019 – In consideration of a sufficient supply of final remedy inflators and upon discussion with the Agency, Toyota
decided to conduct a campaign to replace the air bag inflator or the air bag assembly on certain Toyota
and Lexus vehicles.
January 2, 2019 – Takata filed Part 573 reports in accordance with terms specified in the May 4, 2016 Amendment to the November 3, 2015 Consent Order between Takata and the National Highway Traffic Safety Administration (NHTSA).
January 7, 2020 – Based on Takata’s Part 573 reports, Toyota
decided to conduct a voluntary safety recall to identify the subject vehicles. In doing so, Toyota
does not fully endorse the content of Takata’s Part 573 reports.
- Description of Corrective Repair Action:
All known owners of the affected Toyota
and Lexus vehicles were notified by first class mail to return their vehicles to a Toyota
or Lexus dealer. Depending on the vehicle model, dealers will replace the front passenger or driver airbag inflator or airbag assembly.
Reimbursement Plan for pre-notification remedies
The owner letter instructed vehicle owners who have paid to have this condition remedied prior to this campaign to seek reimbursement pursuant to Toyota
’s General Reimbursement Plan.
- Recall Schedule:
Notifications to owners of the affected vehicles has already occurred. A copy of the draft owner notification letter has been provided to the agency.
- Distributor/Dealer Notification Schedule:
Notifications to distributors/dealers were previously sent on October 17, 2019. Copies of dealer communications have already been submitted to the agency.

- Description:
The subject vehicles are equipped with front passenger or driver air bag inflators as a “like-for-like” replacement under a prior recall which contain a non-desiccated, phase stabilized ammonium nitrate propellant. Under the Amended Consent Order of May 2016 between Takata and NHTSA, Takata is required to make a series of safety defect decisions when the inflators become defective and file Defect Information Reports (DIRs). The scheduled Takata DIRs that affect the subject vehicles will not occur until late 2019. However, in consideration of a sufficient supply of final remedy inflators, and upon discussion with the Agency, Toyota is accelerating the launch of the remedy for these vehicles in advance of the Takata decisions. Toyota will amend this report at the time Takata files its DIR to acknowledge Takata’s defect decision.

Toyota previously advised dealers to discontinue the installation of the non-desiccated parts listed below for recall, customer pay repairs, and over-the-counter sales (if applicable).
At the time of the original notice to discontinue use of these parts Toyota requested dealers to return these parts free of charge. If your dealership still has inventory of any of the part numbers listed below, please remove all pieces from your inventory.
To remove these parts from your inventory, Toyota previously communicated that you should create a manual R8 exception return for these parts, and then return them to your facing PDC for credit. However, Toyota is now asking all dealers to return any parts listed below that still remain in dealer inventory by utilizing the Hazmat Part Return Program. Dealers will be issued a refund for all part returns. This part return program is outlined in PANT Bulletin 2014-042.
Important Note:
In order to return the inflator, packages must NOT be opened/tampered with.
Toyota does NOT accept any of these part numbers on warranty claims. In addition, Toyota recommends that you follow applicable laws prohibiting introduction of recalled parts into commerce, including over-the-counter sales.

Air Bag Inflator Rupture

The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers. All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.
During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers. Nonetheless, at ODI’s insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation. The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures. As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.
Takata’s initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida. Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas. Takata’s testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions. During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.
Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina. Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls. Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity. Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing. All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.
The investigation now includes all manufacturers and vehicles known to be affected at this time. ODI’s investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.
