Transport Hook May Damage Fuel Filler Pipe

Summary
Toyota Motor Engineering & Manufacturing (Lexus) is recalling certain 2019 Lexus RX350 and 2021 RX350 vehicles. The transport hook may not have been removed prior to sale. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 301, “Fuel System Integrity.”
Remedy
Lexus will notify owners, and dealers will inspect and, as necessary, remove the rear transport hook, free of charge. The recall began April 1, 2021. Owners may contact Lexus customer service at 1-800-331-4331. Lexus’ number for this recall is 21LA01.

On February 3, 2021 Lexus filed a Noncompliance Information Report (NCIR) with the National Highway Traffic Safety Administration (NHTSA) informing the agency of our intent to conduct a voluntary Safety (Noncompliance) Recall on certain 2019 and certain 2021 model year RX 350 vehicles.
Condition
A hook, used during shipping, is attached to the left rear corner of these vehicles. This hook is intended to be removed prior to sale but may not have been removed. In certain rear end collisions, this hook may damage a part of the vehicle fuel system and can result in an increased risk of a fire. Thus, these vehicles may not meet certain U.S. safety requirements.
Remedy
For all involved vehicles, any authorized Lexus dealer will inspect to confirm if the left rear transport hook is present and remove it if necessary, FREE OF CHARGE to customers.
Covered Vehicles
There are approximately 1,100 vehicles covered by this Safety (Noncompliance) Recall. There are no vehicles covered by this Safety (Noncompliance) Recall that were distributed to Puerto Rico.
Owner Letter Mailing Date
Lexus will notify owners by early April 2021.
Lexus makes significant effort to obtain current customer name and address information from each state through industry resources when mailing owner letters. In the event your dealership receives a notice for a vehicle that was sold prior to the Safety Noncompliance Recall announcement, it is the dealership’s responsibility to forward the owner letter to the customer who purchased the vehicle.
Please note that only owners of the covered vehicles will be notified. If you are contacted by an owner who has not yet received a notification, please verify eligibility by confirming through TIS prior to performing repairs. Dealers should perform the repair as outlined in the Technical Instructions found on TIS.
Salvage Title Vehicles
Every attempt should be made to complete an open Safety Recall when circumstances permit, unless noted otherwise in the Safety Recall dealer letter.

Approximately 95%. Based on survey results of a non-representative sampling of vehicles in the field in the U.S., Toyota estimates that approximately 95% of the involved vehicles may not have had the involved transport hook removed prior to sale. Whether the issue in each case will lead to an increased risk of fire depends on the specifics of any rear end collision and whether the collision causes an involved transport hook to damage the fuel filler pipe.
Description of Noncompliance:
The transport hook attached to the left rear corner of each of the subject vehicles is used during marine transport. This hook on the subject vehicles is intended to be removed prior to sale but may not have been removed. If a subject vehicle with this transport hook is involved in certain rear end collisions, the fuel filler pipe could become damaged resulting in an increased risk of a fire. As a result, these vehicles may not meet certain requirements of FMVSS No. 301, paragraph S5.6(b).
Test Results and Other Information:
During a port audit, Toyota team members found that the left rear corner transport hooks on certain 2021 RX350 vehicles were not being removed. Subsequent investigation identified that adherence to incorrect processing check sheets at ports and lack of adherence to receiving check sheets at dealer/distributor facilities may have led to these hooks not being removed. Toyota
identified that this issue may have affected certain 2021 RX350 vehicles distributed to the continental U.S. and Alaska, and certain 2019 RX350 vehicles distributed to Hawaii. In parallel, Toyota
confirmed the impact of the presence of this transport hook on vehicle performance in certain rear end collisions, finding that the hook could cause the fuel filler pipe to become damaged in certain rear end collisions for these vehicles. Based on this information, on January 28, 2021, Toyota
decided that the subject vehicles may not meet certain requirements in FMVSS No. 301, paragraph S5.6(b) and decided to perform a field action.
Description of Corrective Repair Action:
For all involved vehicles, dealers will inspect to confirm if the left rear transport hook is present and remove it if necessary, at no cost to customers.
Reimbursement Plan for pre-notification remedies
As the owner notification letters will be mailed well within the active period of the Toyota New Vehicle Limited Warranty (“Warranty”), all involved vehicle owners for this recall would have been provided a repair at no cost under Toyota
’s Warranty.
Recall Schedule:
Notifications to owners of the affected vehicles will occur by April 3, 2021. A copy of the draft owner notification letter(s) will be submitted as soon as available.
Distributor/Dealer Notification Schedule:

AMENDED NON-COMPLIANCE INFORMATION REPORT (21V-043)
This amends Toyota’s Part 573 Report (21V-043) of February 3, 2021 concerning the transport hook issue on certain Lexus RX350 vehicles.
Based on NHTSA’s request, Toyota is adding date information to the “Tests Results and Other
Information” section. In addition, Toyota identified a typographical error in the referenced provision of FMVSS in the Description of Noncompliance section, and has corrected it.
The Description of Non-compliance and Test Results and Other Information sections are revised with the following:
Description of Non-compliance:
The transport hook attached to the left rear corner of each of the subject vehicles is used during marine transport. The repair quality of covered vehicles is extremely important to Lexus. All dealership technicians performing this recall are required to successfully complete the most current version of the E-Learning course “Safety Recall and Service Campaign Essentials”. To ensure that all vehicles have the repair performed correctly; technicians performing this recall repair are required to currently hold at least one of the following certification levels:
• Certified
• Senior
• Master
It is the dealership’s responsibility to select technicians with the above certification level or greater to perform this recall repair. Carefully review your resources, the technician skill level, and ability before assigning technicians to this repair. It is important to consider technician days off and vacation schedules to ensure there are properly trained technicians available to perform this repair at all times.

affected or were previously completed, even by another dealer.
III. PREPARATION
A. PARTS
The part required for this repair (rubber plug) is located in a plastic bag in the vehicle glove
box. If the rubber plug is missing from the glove box, a new one may be ordered and included
on the campaign warranty claim. Refer to page #5 in the TI for the P/N.

a. Look underneath the vehicle in the left rear corner (drivers’ side) for the presence of the LEFT
side Transport Hook.
Note: There will be a Transport Hook installed on the RIGHT side of the vehicle. This is
normal and it should NOT be removed.

VI. REMOVE LEFT SIDE TRANSPORT HOOK
The LEFT side (Driver) Transport Hook will be removed if it’s still installed. The Right side (Passenger) Transport Hook will NOT be removed. ONLY REMOVE THE LEFT SIDE TRANSPORT HOOK.

2. COMPLETE INSTALLATION
a. Re-install the two bolts into their original locations.
Torque: 30 N*m (306 kgf*cm, 22 ft.lbs)
b. Open the glove box of the vehicle and locate the rubber Hole Plug.
c. Install the rubber Hole Plug into the opening that was exposed by the removal of the Transport Hook.
Note: The Hole Plug is located inside the Glove Box of the vehicle. If the Hole Plug is missing, a new one can be ordered (part 90950-01808).

Bring the rubber plug, originally placed in your glove box, with you to the dealership
During the inspection, if the dealer confirms that the left rear transport hook is present on your vehicle, the dealer will remove it. During the removal
process, the dealer will install a small rubber plug (about 1” diameter) into an opening on the vehicle body. When the vehicle was originally sold, this
rubber plug was located inside the glove box of each vehicle. If available, Lexus recommends that you bring this rubber plug with you when you visit
the dealership to have the remedy performed. This will help reduce any inconvenience to you.
If you no longer have this plug, advise the dealership when scheduling your appointment so that the dealership may order the plug ahead of time.
You will NOT BE CHARGED for the plug. The dealer will perform this remedy FREE OF CHARGE to you.

On February 3, 2021 Lexus filed a Noncompliance Information Report (NCIR) with the National Highway Traffic Safety Administration (NHTSA) informing the agency of our intent to conduct a voluntary Safety (Noncompliance) Recall on certain 2019 and certain 2021 model year RX 350 vehicles.
Condition
A hook, used during shipping, is attached to the left rear corner of these vehicles. This hook is intended to be removed prior to sale but may not have been removed. In certain rear end collisions, this hook may damage a part of the vehicle fuel system and can result in an increased risk of a fire. Thus, these vehicles may not meet certain U.S. safety requirements.
Remedy
For all involved vehicles, any authorized Lexus dealer will inspect to confirm if the left rear transport hook is present and remove it if necessary, FREE OF CHARGE to customers.
Covered Vehicles
There are approximately 1,100 vehicles covered by this Safety (Noncompliance) Recall. There are no vehicles covered by this Safety (Noncompliance) Recall that were distributed to Puerto Rico.
Owner Letter Mailing Date
Lexus will notify owners by early April 2021.
Lexus makes significant effort to obtain current customer name and address information from each state through industry resources when mailing owner letters. In the event your dealership receives a notice for a vehicle that was sold prior to the Safety Noncompliance Recall announcement, it is the dealership’s responsibility to forward the owner letter to the customer who purchased the vehicle.
Please note that only owners of the covered vehicles will be notified. If you are contacted by an owner who has not yet received a notification, please verify eligibility by confirming through TIS prior to performing repairs. Dealers should perform the repair as outlined in the Technical Instructions found on TIS.

Dealer Inventory Procedures
New Vehicles in Dealership Inventory
There are approximately 142 vehicles in new dealer inventory as of February 2, 2020.
Under Title 49, Section 30112 of the United States Code, a dealer cannot sell, offer for sale, or introduce or deliver for introduction in interstate commerce a new motor vehicle when it is aware that the vehicle does not comply with an applicable Federal Motor Vehicle Safety Standard or contains a defect related to motor vehicle safety. Further, 49 Code of Federal Regulations §577.13 requires us to provide the following advisory: It is a violation of Federal law for a dealer to deliver a new motor vehicle or any new or used item of motor vehicle equipment (including a tire) covered by this notification under a sale or lease until the defect or noncompliance is remedied.
Lexus reimburses dealers for flooring costs associated with new vehicles held in dealership inventory until the recall remedy becomes available. Lexus provides these flooring reimbursements at 60-day intervals. Lexus reserves the right to withhold or charge-back any flooring reimbursement funds that may have been paid to the dealer if it determines that a new vehicle subject to a safety recall was sold without first being remedied.
Vehicle Safety Recall completion should always be verified through TIS. We request your assistance to ensure involved vehicles are identified and not delivered prior to performing the remedy.
NOTE:
• New vehicles in dealer inventory subject to a Safety Recall must be remedied before delivery into commerce under a sale or lease. Dealer-to-dealer trades as well as vehicle sales between dealers should not be made until a vehicle is remedied.
• Dealers can identify if any of their new and used inventory has any open campaigns in the Vehicle Inventory Summary available in Dealer Daily (https://dealerdaily.lexus.com/). The Vehicle Inventory Summary may take up to 4 hours to populate information for newly launched campaigns.
Inspection Reminder Mirror Hang Tags for Covered Vehicles in New Dealer Stock
To easily recognize vehicles involved in this Safety (Noncompliance) Recall, each dealership should utilize Inspection Reminder Hang Tags. Inside the vehicle’s glove box are stickers containing the VIN. Apply one of these stickers to the hang tag and install the hang tag in the vehicle. A sample copy of the Inspection Reminder Hang Tag has been included for your reference.

Pre-Owned Vehicles in Dealer Inventory
To ensure customer satisfaction, Lexus requests that dealers complete this Safety (Noncompliance) Recall on any used vehicles currently in dealer inventory that are covered by this Safety (Noncompliance) Recall prior to customer delivery. However, if the campaign cannot be completed (for example, due to remedy parts availability), delivery of a covered vehicle is acceptable if disclosed to the customer that the vehicle is involved in a Safety Recall.
Lexus expects dealers to use the attached Customer Contact and Vehicle Disclosure Form to obtain vehicle buyer information. Dealers are expected to provide a copy of the completed form, along with the most current FAQ, to the vehicle buyer. Lexus and the dealer may use this information to contact the customer when the remedy becomes available.
Keep the completed form on file at the dealership and send a copy to quality_compliance@toyota.com. In the subject line of the email state “Disclosure Form 21LA01” and include the VIN.
NOTE: Dealers can identify if any of their new and used inventory has any open campaigns in the Vehicle Inventory Summary available in Dealer Daily (https://dealerdaily.lexus.com/). The Vehicle Inventory Summary may take up to 4 hours to populate information for newly launched campaigns.
L/Certified Vehicles
L/Certified policy prohibits the certification of any vehicle with an outstanding Safety Recall, Special Service Campaign, or Limited Service Campaign. Thus, no affected units are to be designated, sold, or delivered as L/Certified until all applicable Safety Recalls, Special Service Campaigns, and Limited Service Campaigns have been completed on that vehicle.
LCCS Service Loaners
Lexus requests that dealers remove all LCCS Service Loaner vehicles from service that are covered by a Safety Recall unless the defect has been remedied.
Customer Handling, Parts Ordering, and Remedy Procedures
Customer Contacts
Customers who receive the owner letter may contact your dealership with questions regarding the letter and/or the Safety Noncompliance Recall. Please welcome them to your dealership and answer any questions that they may have. A Q&A is provided to assure a consistent message is communicated.
Customers with additional questions or concerns are asked to please contact the Lexus Brand Engagement Center (1-800-255-3987) – Monday through Friday, 7:00 am to 7:00 pm, Saturday 8:00 am to 5:00 pm Central Time.
Head Unit Notifications
Head unit notifications are electronic messages that are displayed in the vehicle’s audio system screen. Customers who receive head unit notification regarding this Safety (Noncompliance) Recall are requested to schedule an appointment with their authorized dealer to have this Safety (Noncompliance) Recall completed.
When these messages are received by the vehicle’s Data Communication Module, the head unit will display a pop-up prompt stating that the vehicle has a new Safety (Noncompliance) Recall. The prompt will contain options to ‘Snooze’ or to ‘View’ the message. If a customer chooses ‘Snooze’, the message will continue to reappear every 20 minutes until the customer chooses ‘View’. If a customer views and then closes the message, the message will be available in the Notification App if the customer chooses to review it again. If the vehicle’s completion status remains incomplete for a period of 90 days from the message being viewed, the head unit may display a renotification pop-up prompt as an additional reminder to the customer to have this Safety (Noncompliance) Recall completed.
The message will completely clear from the vehicle once the following conditions are met: The Safety (Noncompliance Recall) is completed, the dealer has filed a claim, and the claim is approved by Lexus. Then the message will be cleared at the next clearing cycle, which currently happens weekly.
Owners who receive a head unit notification after having this Safety (Noncompliance) Recall completed can be advised to ignore the message. Owners with additional concerns can be directed to the Lexus Brand Engagement Center (1-800-255-3987) – Monday through Friday, 7:00 am to 7:00 pm, Saturday 8:00 am to 5:00 pm Central Time.

Salvage Title Vehicles
Every attempt should be made to complete an open Safety Recall when circumstances permit, unless noted otherwise in the Safety Recall dealer letter.
For complete details on this policy, refer to Lexus Warranty Policy 4.15, “What Is Not Covered by The Lexus New Vehicle Limited Warranty”.

Parts Ordering Process –
The part required for this repair (rubber plug) is located in a plastic bag in the vehicle glove box. If the rubber plug is missing from the glove box, a new one may be ordered and included on the campaign warranty claim. Refer to page #5 in the Technical Instructions for the P/N.
It is possible that parts for this campaign are either required to be ordered in Campaign Part Order Request (CPOR) on Service Lane or have been placed on Manual Allocation Control (MAC) due to potential limited part availability. Please check the CPOR/MAC report on Dealer Daily for the most up-to-date parts ordering information. Dealers can identify which parts ordering method to use by reviewing the parts information section of Dealer Daily and checking for a MAC code on the part numbers below. For MAC code C, order through CPOR. For MAC code D, refer to the MAC report for further instructions.
Technician Training Requirements
The repair quality of covered vehicles is extremely important to Lexus. All dealership technicians performing this repair are required to successfully complete the most current version of the E-Learning course “Safety Recall and Service Campaign Essentials”. To ensure that all vehicles have the repair performed correctly; technicians performing this repair are required to currently hold at least one of the following certification levels:
• Certified
• Senior
• Master
Always check which technicians can perform the repair by logging on to https://www.LCTPReports.com.. It is the dealership’s responsibility to select technicians with the above certification level or greater to perform this repair. Carefully review your resources, the technician skill level, and ability before assigning technicians to this repair. It is important to consider technician days off and vacation schedules to ensure there are properly trained technicians available to perform this repair at all times.
Remedy Procedures
Refer to TIS for Technical Instructions on repair. Conduct all non-completed Safety Recalls and Service Campaigns on the vehicle during the time of appointment.
Repair Quality Confirmation
The repair quality of covered vehicles is extremely important to Lexus. To help ensure that all vehicles have the repair performed correctly, please designate at least one associate (someone other than the individual who performed the repair) to verify the repair quality of every vehicle prior to customer delivery.
Parts Recovery Procedures
All parts replaced as part of this Safety Recall must be turned over to the parts department until appropriate disposition is determined. The parts department must retain these parts until notification via the Parts Recovery System (PRS) is received indicating whether to ship or scrap the parts. These parts are utilized by various departments for defect analysis, quality control analysis, product evaluation, as well as other purposes.
To help minimize dealer storage challenges, Lexus recommends that dealers:
• File the campaign claim accurately and promptly. The time a dealer is required to hold parts is based on when the campaign claim is paid by Lexus.
• Monitor the Warranty Parts Recovery Notifications and Part Scrap Report regularly.
