Summary

ToyotaeBay Motor Engineering & Manufacturing (ToyotaeBay) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 ScioneBay xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles.

These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

 

Remedy

ToyotaeBay will notify owners. Depending on the vehicle model, dealers will replace the driver or front passenger air bag inflator or the air bag assembly using an alternate inflator , free of charge. The recall is expected to begin November 15, 2019. Owners may contact ToyotaeBay customer service at 1-888-270-9371 or Lexus customer service at 1-800-255-3987. ToyotaeBay’s numbers for this recall are J0A, J0B, and J0C. Lexus’ numbers for this recall are JLI, JLJ, and JLK.

Q1:What is the condition?
A1:The subject vehicles are equipped with front passenger or front driver airbag inflators produced by Takata. The propellant in these inflators may degrade after prolonged exposure to high absolute humidity and fluctuating high temperatures.  Degraded propellant can cause the inflator to explode during airbag deployment.  In the event of an inflator explosion, metal fragments could pass through the airbag cushion material, striking the vehicle occupants, and result in serious injury or death.

 

Q2:Why is Takata recalling all frontal airbag inflators manufactured with propellant that do not also include a desiccant?
A2:Multiple independent investigations and NHTSA’s independent expert concluded that the propellant in non-desiccated, frontal Takata airbag inflators degrades over time.  The degradation is the result of various factors existing in the location where the vehicle containing the inflators is principally operated, including long-term exposure to environmental moisture and fluctuating high temperatures.

Q2a: What is the Inflator?
A2a: The inflator is a device contained within the airbag assembly.  It contains solid propellant wafers which are ignited in the event airbag deployment is triggered.  When ignited, the wafers expand into an inert gas, inflating the airbag.

 

Q2b: What happens when the non-desiccated propellant degrades?
A2b: If the propellant degrades substantially, the inflators can become over-pressurized and explode during airbag deployment.

 

Q2c: What is a desiccant?
A2c: A desiccant is a material that acts as a drying agent.  Desiccants collect moisture present in their immediate environment, helping to limit moisture absorption by other nearby materials, such as propellant in an airbag inflator.

 

Q2d: What is absolute humidity? 
A2d: The measure of the water vapor content in the air is known as absolute humidity, and it is displayed in grams of water vapor per cubic meter of air. Higher temperature, southern coastal climates consistently experience the greatest concentrations of water vapor in the air, as warmer ambient air can hold more water.

Note: relative humidity is simply a percentage value and is related to current or measured temperature; therefore, areas with high relative humidity do not necessarily have high absolute humidity.

 

Q2e: How is humidity related to the condition?
A2e: The specific relationship of humidity to the operation of the inflator and the cause of improper airbag inflator performance and explosion is still under investigation.

 

 

Q3:I already had my recall repair completed under a previous recall of the passenger airbag inflator. 

Why is my vehicle being recalled again?

A3:All vehicles repaired under a previous recall with a remedy part containing Phase Stabilized Ammonium Nitrate (PSAN) without desiccant are scheduled for recall under the Takata Coordinated Remedy Program administered by the National Highway Traffic Safety Administration (NHTSA) beginning December 31, 2019.  Vehicles repaired using a PSAN inflator without desiccant are sometimes referred to as “Like for Like” vehicles.

Q3a: If these “Like for Like” vehicles are not scheduled for recall until later on, why is ToyotaeBay recalling these vehicles now?
A3a: Due to sufficient supply of remedy parts and dealer capacity for certain vehicles, ToyotaeBay was able to pull ahead the launch of the remedy phase for these vehicles involved in advance of Takata’s decision.

 

Q3b: When is ToyotaeBay planning to recall other vehicles that may be involved in this “Like for Like” phase?
A3b: There are no other vehicles or future phases of “Like for Like”.

 

 

Q4:Which airbags in ToyotaeBay vehicles are affected by this field action?
A4:This field action only includes certain ToyotaeBay vehicles repaired under a previous recall with Takata produced front passenger or driver airbag inflators that do not include a desiccant.

 

Q5: Are there concerns with other airbags in the vehicle?
A5:No.  This recall only applies to certain front passenger or driver airbag inflators manufactured and installed in certain ToyotaeBay vehicles.  Other airbags in the vehicle are not affected by this Safety Recall.

Q1: Why is ToyotaeBay providing unrepaired VINs to dealers?

ToyotaeBay is making every effort to increase the completion rate for the Takata recall.  Engaging dealerships in this effort is of vital importance.  ToyotaeBay recognizes that dealers have local expertise for their market areas and need to know what unrepaired vehicles are in their area in order to provide assistance in our efforts.
Q2: How will ToyotaeBay provide unrepaired VINs to dealers?
A2:

 

ToyotaeBay is developing an integration within the Service Opportunity Access for Retention (SOAR) system to provide unrepaired VINs and customer contact information to dealers.

Q2a: What if my dealership doesn’t use SOAR today?
A2a: At this time, we can only provide this data through the SOAR system.  However, SOAR is free to access for all dealers.  If your dealership would like to obtain access, find and click the SOAR link on the Service menu in Dealer Daily and follow the instructions to sign up.  Note that SET dealers are not involved in the SOAR program.

 

Q2b: What if I don’t have access to SOAR?
A2b: If your dealership is not a current SOAR user, please contact the Dealer Daily administrator within your dealership to gain access.  Only users who have a business need to access the data within SOAR should be provided access.

 

 

Q3: Are there any special conditions or terms of use involved with this data?
A3:Yes, each time a dealer accesses the data in SOAR, they will be required to agree to a series of terms and conditions.  The use of this data is strictly prohibited for any purpose other than contacting customers about the incomplete recall on their vehicle.  It cannot be used for marketing of any kind.

 

Q4: Can I download the data and provide to members of my dealership to reach out to customers?
A4:Yes, we encourage you to use the data to reach customers in your area for the purpose of informing them about an open recall.  We ask that you keep in mind that Terms and Conditions state that you may not load this data into any other database and you may not retain the data for longer than is necessary to reach out to customers for that purpose.

 

Q5: Will I be able to use my OCPe National Business Partner (Epsilon, AutoPoint, Gulf States Marketing) to reach out to customers in my area?

Q5a: Can I provide the data to other Third-Party Vendors?
A5a: Yes, but any Third-Party Vendor which receives this data must be contractually bound in writing to the same Terms and Conditions which dealers agree to when accessing the data.

 

Q6: Will ToyotaeBay purge the list of unrepaired VINs in my area as they are repaired?
A6:Yes, each night SOAR will be purged of any VINs for which repair warranty claims were received on the previous day.

 

Q7: What data will be available in SOAR?
A7:VIN, customer name, address, phone number, and email address.  Note that phone number and email address may not be available for all VINs.

 

Q8: What can I send to customers to notify them about their open recall?
A8:ToyotaeBay has created specific templates.  Dealers MUST use these templates for all communications.  These communications have been designed so that dealers can incorporate their own logo, contact information, and links to online scheduling systems.

Q8a: Where can I obtain the communication templates?
A8a: Communication templates will be available within SOAR for download.

 

Q8b: Can I change the content of the templates?
A8b: No. These specific templates have been reviewed as part of the National Highway Traffic Safety Administration’s (NHTSA) Coordinated Remedy Program.  Consistent messaging in communications about the Takata recalls is important.  Dealers should not change the content of the template aside from adding their dealer logo and contact information.  Links to online appointment scheduling can be added as well as service hours if desired by the dealer.

 

 

Q9: What are the terms and conditions of use?
A9:Terms and conditions are shown below.  Dealers will be required to agree to these terms and conditions each time they access this data on VINs with unrepaired Takata inflators in SOAR.  Failure to adhere to any of these terms can result in being denied access to unrepaired VIN information.

Why am I receiving another recall notice if I already had the recalled inflator replaced?

ToyotaeBay previously recalled these vehicles for this same condition. At that time, the inflator used as a replacement was a new one of the same design (so this was a “like for like” replacement). However, the replacement inflator now in your vehicle can still degrade over time as noted above and must be replaced.

 

What will ToyotaeBay do?

ToyotaeBay dealers will replace the front passenger airbag inflator or airbag assembly, depending on the model, with one of improved design FREE OF CHARGE to you.

 

This is an important Safety Recall.

The remedy will take approximately one to three and a half hours. However, depending on the dealer’s work schedule, it may be necessary to make your vehicle available for a longer period of time.

ToyotaeBay strongly recommends that you have this Safety Recall remedy performed immediately. Please contact any authorized ToyotaeBay dealer to schedule an appointment to have the remedy performed. This notice is sent to you in accordance with the National Traffic and Motor Vehicle Safety Act. ToyotaeBay has decided that a defect, which relates to motor vehicle safety, exists in certain 2004 and 2005 RAV4 vehicles.

You received this notice because our records, which are based primarily on state registration and title data, indicate that you are the current owner.

 

What is the condition?

The subject vehicles are equipped with front driver airbag inflators produced by Takata. The propellant in these inflators may degrade after prolonged exposure to high absolute humidity and fluctuating high temperatures. Degraded propellant can cause the inflator to explode during airbag deployment. In the event of an inflator explosion, metal fragments could pass through the airbag cushion material, striking the vehicle occupants, and result in serious injury or death.

 

What Should You Do?

Please contact any authorized ToyotaeBay dealer to schedule an appointment to have the remedy performed as soon as possible. The remedy will require parts replacement. We recommend you contact your dealer to schedule an appointment in advance to confirm parts availability and minimize your inconvenience. Why am I receiving another recall notice if I already had the recalled inflator replaced?

ToyotaeBay previously recalled these vehicles for this same condition. At that time, the inflator used as a replacement was a new one of the same design (so this was a “like for like” replacement). However, the replacement inflator now in your vehicle can still degrade over time as noted above and must be replaced.

 

What will ToyotaeBay do?

ToyotaeBay dealers will replace the front driver airbag inflator with one of improved design FREE OF CHARGE to you.

 

This is an important Safety Recall.

The remedy will take approximately one hour. However, depending on the dealer’s work schedule, it may be necessary to make your vehicle available for a longer period of time.

ToyotaeBay strongly recommends that you have this Safety Recall remedy performed immediately. Please contact any authorized ToyotaeBay dealer to schedule an appointment to have the remedy performed. AMENDED DEFECT INFORMATION REPORT
(19V-741)

 

This supplements ToyotaeBay’s Part 573 Report (19V-741) concerning the front passenger air bag assembly issue on certain ToyotaeBay and Lexus vehicles.

 

The subject recall affects certain ToyotaeBay and Lexus vehicles equipped with front passenger or driver air bag inflators as a “like-for-like” replacement under a prior recall which contain a non-desiccated, phase stabilized ammonium nitrate propellant.  After the amended DIR filing on March 9, 2020, ToyotaeBay has developed effective methods to replace only the affected sub-components of the air bag assembly.  Thus, ToyotaeBay is supplementing its Part 573 Report (19V-741) to include the option, where applicable, for replacement of only the front passenger airbag sub-assembly and inflator.

 

The Description of Corrective Repair Action is revised as noted below:

The total number of vehicles potentially involved (increase) are revised as noted below.  The production period of each involved vehicle are not affected by this change.

  1. Percentage of Vehicles Estimated to Actually Contain the Defect:

Toyota is unable to provide an estimate of the percentage of vehicles to actually contain the defect.  As noted in the May 2016 Amendment to the Takata Consent Order, the length of time that a non-desiccated frontal Takata PSAN inflator will have reached the point where they pose an unreasonable risk to safety could be quite long and is dependent on the age of the inflator and environmental exposure, among other factors.

 

  1. Description:

The subject vehicles are equipped with front passenger or driver air bag inflators as a “like-forlike” replacement under a prior recall which contain a non-desiccated, phase stabilized ammonium nitrate propellant.  According to the Part 573 Reports submitted by Takata on January 2, 2020, a defect related to motor vehicle safety may arise in the inflators due to propellant degradation occurring after prolonged exposure to high absolute humidity, high temperatures, and high temperature cycling.  Activation of a non-desiccated ammonium nitrate inflator with degraded propellant may result in an inflator rupture.  An inflator rupture may cause metal fragments to pass through the airbag and into the vehicle interior at high speed, which may result in injury or death to vehicle occupants.  In consideration of a sufficient supply of final remedy inflators, and upon discussion with the Agency, Toyota previously accelerated the launch of the remedy for these vehicles in advance of the Takata decisions under this Recall 19V-741.  Takata’s January 2, 2020 report prompts this amendment for certain Toyota and Lexus vehicles.

 

  1. Chronology of Principal Events:

October 11, 2019 – In consideration of a sufficient supply of final remedy inflators and upon discussion with the Agency, Toyota decided to conduct a campaign to replace the air bag inflator or the air bag assembly on certain Toyota and Lexus vehicles.

 

January 2, 2019 – Takata filed Part 573 reports in accordance with terms specified in the May 4, 2016 Amendment to the November 3, 2015 Consent Order between Takata and the National Highway Traffic Safety Administration (NHTSA).

 

January 7, 2020 – Based on Takata’s Part 573 reports, Toyota decided to conduct a voluntary safety recall to identify the subject vehicles.  In doing so, Toyota does not fully endorse the content of Takata’s Part 573 reports.

 

  1. Description of Corrective Repair Action:

All known owners of the affected Toyota and Lexus vehicles were notified by first class mail to return their vehicles to a Toyota or Lexus dealer.  Depending on the vehicle model, dealers will replace the front passenger or driver airbag inflator or airbag assembly.

 

Reimbursement Plan for pre-notification remedies

The owner letter instructed vehicle owners who have paid to have this condition remedied prior to this campaign to seek reimbursement pursuant to Toyota’s General Reimbursement Plan.

Notifications to owners of the affected vehicles has already occurred.  A copy of the draft owner notification letter has been provided to the agency.

Not applicable at this time.

 

  1. Description:

The subject vehicles are equipped with front passenger or driver air bag inflators as a “like-for-like” replacement under a prior recall which contain a non-desiccated, phase stabilized ammonium nitrate propellant.  Under the Amended Consent Order of May 2016 between Takata and NHTSA, Takata is required to make a series of safety defect decisions when the inflators become defective and file Defect Information Reports (DIRs).  The scheduled Takata DIRs that affect the subject vehicles will not occur until late 2019.  However, in consideration of a sufficient supply of final remedy inflators, and upon discussion with the Agency, Toyota is accelerating the launch of the remedy for these vehicles in advance of the Takata decisions.  Toyota will amend this report at the time Takata files its DIR to acknowledge Takata’s defect decision.

 

  1. Chronology of Principal Events:

October 11, 2019 – In consideration of a sufficient supply of final remedy inflators and upon discussion with the Agency, Toyota decided to conduct a campaign to replace the air bag inflator or the air bag assembly on certain Toyota and Lexus vehicles.

 

  1. Description of Corrective Repair Action:

All known owners of the affected Toyota and Lexus vehicles will be notified by first class mail to return their vehicles to a Toyota or Lexus dealer.  Depending on the vehicle model, dealers will replace the front passenger or driver airbag inflator or airbag assembly.

 

Reimbursement Plan for pre-notification remedies

The owner letter will instruct vehicle owners who have paid to have this condition remedied prior to this campaign to seek reimbursement pursuant to Toyota’s General Reimbursement Plan.

 

  1. Recall Schedule:

Notifications to owners of the affected vehicles will begin by Mid-November, 2019.  A copy of the draft owner notification letter will be submitted as soon as available.  It will be in accordance with the provisions of Part 577 even though Takata’s defect decision for these vehicles has not yet been made.

 

  1. Distributor/Dealer Notification Schedule:

Notifications to distributors/dealers will be sent on October 17, 2019.  Copies of dealer communications will be submitted as they are issued.

Air Bag Inflator Rupture

The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers. All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.

During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers. Nonetheless, at ODI’s insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation. The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures. As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.

Takata’s initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida. Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas. Takata’s testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions. During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.

Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina. Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls. Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity. Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing. All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.

The investigation now includes all manufacturers and vehicles known to be affected at this time. ODI’s investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.
